On Job Injuries
When an employee of Facilities Management has an injury while working it must be reported to the Worker's Care and a form submitted to Facilities Managementwithin 24 hours. This can be accomplished on the accompanying form.
Motor Vehicle Accidents
If a Facilities Management employee has a accident with a University Vehicle the UK Police must be notify and an accident report filed out by them. It must also be reported to Facilities Management within 24 hours. This can be accomplished on the accompanying form.
Incident Reporting
This form is used for reporting any incident which does not require medical treatment, reporting to Workers' Care, vehicle accidents but does cover any incident which does result in any amount of damage to buildings or equipment..
New Chemical Reporting
When a new product is introduced into the workplace it is required a Safety Data Sheet (SDS) is available. In order to meet these requirements information must be reported. This SDS can be obtained and made available for Facilities Management employees' use.
Voluntary Use Respirator
This form is used for individuals requesting the voluntary use of a respirator. This form is required in accordance with the OSHA standard 29CFR1910.134 Appendix D.
Hot Work Notification
Prior to starting an operation when a Hot Work Permit will be required, it is necessary to submit a notification to the appropriate individuals.
